1. The tuition fee payments will start from Sep 1st.

2. Last Month Fees Payment will be charged in Advance upon registration.

3. The students will be placed on a wait list if the enrollment reaches it's capacity.

Notes:

I hereby authorize Hamilton Downtown Islamic School to initiate automatic deposits to my account at the financial institution named above. I also authorize Hamilton Downtown Islamic School to make withdrawals from this account in the event that a credit entry is made in error. Further, I agree not to hold Hamilton Downtown Islamic School responsible for any delay or loss of funds due to incorrect or incomplete information supplied by me or by my financial institution or due to an error on the part of my financial institution in depositing funds to my account. This agreement will remain in effect until Hamilton Downtown Islamic School receives a written notice of cancellation from me or my financial institution, or until I submit a new direct deposit form to Hamilton Downtown Islamic School.

I Take full responsibility to enroll my Child at Hamilton Downtown Islamic School.
